Appointments
If you can’t keep your scheduled appointment for any reason or know you’ll be delayed, please call and let us know as soon as possible. If an appointment is canceled less than 24 hours before the scheduled time, a fee may be assessed.
Insurance
We participate with most all insurance plans including: Aetna, BCBS, BCBS Federal, BCBS State, Boeing, Blue Choice, Cigna, First Health, Great West, Humana, Medicaid, Medicare, PAI, Premier, Tricare, United Healthcare. If your insurance company isn’t listed here, please call and we’ll check to make sure we accept your provider. If you don’t have, or don’t wish to use, insurance and would prefer to self-pay, check out our OPT Program.
